I have read many, many times of the loyalty oath that one had to sign to be reinstated as a "good" citizen but had never before seen one, now I have!

He spent the rest of his time in Camp Chase Prison Ohio which is where he signed the oath. Notice at the bottom that is says 'under the penalty of death."
Seems they were kinda serious back then on loyalty oaths.
