heifer is pretty much another name for cow, which means a bigger person when referring to a human in that term. A skinny heifer? Besides being a tasty dessert, has no real connotation or substance. It means a skinny cow. Whether it was meant as an insult or not depends on the context. And of course whether you take anything as an insult depends entirely on you.

If it is an insult, it is born of envy and has more to do with the offender's insecurities than anything about you.
I would take it as meaning a skinny person who enjoys eating a lot or whatever they want...someone with an insane metabolism that people tend to be jealous of.
